    Please fill this form if you have COVID symptoms, exposure or a positive test.

  • Symptoms may appear 2-14 days after exposure to the virus. People with these symptoms may have COVID-19: Fever or chills, Cough, Shortness of breath or difficulty breathing, Fatigue, Muscle or body aches, Headache, New loss of taste or smell, Sore throat, Congestion or runny nose, Nausea or vomiting, Diarrhea.

  • You must quarantine due to being exposed to COVID19 by:

    1. Being within close contact with some one who has tested positive for COVID19 (14 Days),
    2. If one of the household members is being evaluated for COVID-19, the rest of the house hold must be quarantined until an alternative diagnosis is made or negative result received (14 Days).
    3. You are living with someone who has tested positive for COVID19 (14 Days)
    4. You tested positive for COVID19 (10 Days with 24 hours without symptoms or the aide of medicine including over the counter).
  • Individual who has had close contact (< 6 feet) for ≥15 minutes:

    • Person with COVID-19 who has symptoms (in the period from 2 days before symptom onset until they meet criteria for discontinuing home isolation; can be laboratory-confirmed or a clinically compatible illness)
    • Person who has tested positive for COVID-19 (laboratory confirmed) but has not had any symptoms (in the 2 days before the date of specimen collection until they meet criteria for discontinuing home isolation).

    • If you test positive, know what protective steps to take to prevent others from getting sick.
    • If you test negative, you probably were not infected at the time your sample was collected. The test result only means that you did not have COVID-19 at the time of testing. Continue to take steps to protect yourself.
